Gloucestershire Police were called to the B4425 at Riversmeet North near Aldsworth at around 12.45pm after a report of a multi-vehicle collision.
A man in his 50s from Cirencester was pronounced dead at the scene. His next of kin has been informed.
Meanwhile, three people were taken to hospital in Bristol or Oxford with serious injuries, whilst another eight were taken to Gloucester and Cheltenham.
An 18-year-old woman was arrested on suspicion of causing death by dangerous driving, and has been taken to hospital as a precaution.
The crash, which occured between the Coln St Aldwyns and Eastington turnings, involved a grey Mini Cooper, black Audi A3, grey Mitsubishi Polestar 2 and a black Seat Leon.
The B4425 and a number of surrounding roads remained closed for several hours while a collision investigation continued.
A spokesperson for Gloucestershire Police said: "Investigating officers would like to hear from anyone who has not already spoken to police and has information they feel may be relevant.
"Anyone who has dashcam or CCTV footage which they believe would be relevant is also asked to make contact.
"Information can be submitted to police online quoting incident 251 of 2 December."
